SummaryThe Clinical Care Coordinator (CCC) for Mary Free Bed at Covenant is responsible for the overall operation of the rehabilitation hospital and Transitional Care Unit (TCU), and has 24‑hour accountability for all patient care activities and staff who work under his/her direction. The CCC is responsible for organizing, directing, and ensuring effective operations of the rehabilitation hospital and TCU for the assigned shift, taking into account the effect of decisions on the 24‑hour operation of the units.
EssentialJob Responsibilities
- Quality Nursing Care: The Clinical Care Coordinators' primary focus is the delivery of quality nursing care. This includes working with all staff to assure that nursing care is delivered consistently according to established policies, procedures, and protocols; creating equitable staffing assignments; mentoring new staff; ensuring nursing presence at and preparedness for team conferences; communicating with, and following up with physicians; safety monitors;
timely follow‑up on occurrence reports; and customer service recovery as needed. - Review of Nursing Policies: The Clinical Care Coordinator reviews and provides input to the Nurse Manager for nursing policies and procedures, nursing standards of patient care, treatment, and services, and standards of nursing practice; addresses non‑compliance with policies and procedures; and validates key nursing tactics.
- Key Performance Indicators: The Clinical Care Coordinator understands performance measures within the Rehabilitation Hospital and collaborates with staff to identify opportunities for improvement and potential action plans. When process, policy, or procedure changes are developed by the Program Director or Nurse Manager, the CCC directs staff to the information, assists with implementation, ensures staff complete required competencies, and provides feedback to the Nurse Manager.
- Quality Improvement: The Clinical Care Coordinator participates in Mary Free Bed at Covenant's quality improvement program and works with staff to provide excellent rehabilitation nursing care through innovative ideas that align with evidence‑based policies and procedures.
- Patient Safety: The Clinical Care Coordinator plays an important role in providing a safe environment for all patients and staff. As a clinical resource, the CCC evaluates the risks for each patient and, as a patient advocate, ensures appropriate steps are taken to protect patient safety. The CCC also serves as the primary clinical support for employees and responds to all medical emergencies, per policy, as well as all codes (fire, tornado, violence, abductions) within the campus.

The CCC maintains effective interpersonal communication and working relationships with all leaders, staff, physicians, and other key stakeholders; participates actively in interdisciplinary program meetings to evaluate and improve care delivered at Mary Free Bed at Covenant. - Accreditation and Regulatory Responsibilities: The CCC assists the Nurse Manager and Program Director with implementing policies, procedures, and processes that align with regulatory, accreditation, and state hospital standards; serves as a resource for staff in these areas.
