Position: RN HOSPICE WEEKEND FACILITATOR - COVENANT AT HOME

Covenant At Home Hospice RN Weekend Facilitator

The Covenant at Home Hospice RN Weekend Facilitator will be responsible to perform Quality Assurance activity as assigned, will receive and coordinate information from incoming calls for service, will facilitate hospice clinical needs, visits, admissions, and referrals three days weekly from 7a-7p. This includes every Saturday and Sunday as well as alternating additional day during the week each week. This position must consistently exercise discretion and judgment and refers to other community resources as necessary.

Responsibilities

Assists with coordination of patient care with other members of the Hospice and Home Care team. Provides problem solving skills to assist with the immediate issue and coordination of ongoing care. Interfaces with other units to maintain positive team relations. Role models professional nursing behaviors consistent with Covenant Health Care Mission, Vision and Values. Represents agency to the public in a positive cooperative manner.

Answers inquiries for all services. Triages care requests and makes patient visits as scheduled and necessary. Completes appropriate/necessary documentation in a timely fashion. Communicates identified issues/visit reason with the Case Manager. Updates POC as necessary. Demonstrates understanding of Hospice philosophy, hospice symptom management techniques and comfort care. Demonstrates understanding of the hospice levels of care and when to initiate a change.

Demonstrates understanding of hospice regulations and requirements. Functions with awareness of and application of safety issues as identified within the institution. Demonstrates appropriate and professional communication with staff, physicians and ancillary departments to solve problems in patient care delivery. Demonstrates an understanding of appropriate emergency responses and the ability to remain calm. Acts as a liaison between physician and RN (verbal order, follow up calls).

Coordinates information received from patients/families with staff providing care. Completes hospice intake process for incoming referrals. Responsible for special projects. Triages and supports calls and questions from Palliative Care patients Performs CQI activities as assigned. Supports organizational policies. Keeps the manager/coordinator informed of patient care (call events) and notes recommendations on Quality Assurance issues. Maintains records in accordance with agency requirements.

Participates in development, revision and implementation of policies and procedures. Assumes responsibility for personal growth. Participates in education meetings and IDT as required. Demonstrates excellent customer service. Maintains confidentiality with all information in the appropriate manner. Contributes to organization success targets for patient satisfaction and net operating margins. Other duties as assigned.

Qualifications

Current Registered Nurse license in the State of Michigan required.

Bachelor of Nursing degree preferred.

Hospice experience required.

Two (2) years of hospice experience in an accredited and certified agency preferred.

Quality Assurance experience preferred.

Knowledge of current nursing practice.

Customer service focus.

Able to operate with minimal supervision.

Knowledgeable regarding availability of community resources.

Valid driver's license.

Excellent communication skills, both verbal and written.

Computer knowledge, both Covenant at Home and software used within the Covenant at Home system.

Demonstrates problem solving skills.

Ability to type.

Reliable transportation and proof of insurance.

Ability to maintain regular, punctual attendance consistent with the ADA, FMLA and other federal, state and local standards.

Frequent standing, walking, talking, hearing, and tasting/smelling.

Frequent near, midrange and far vision, depth perception, visual accommodation, color vision and field of vision.

Frequent lifting up to 50 lbs.
