Optimal Care is seeking a Home Health Registered Nurse to deliver skilled nursing care in patients' homes, supporting recovery and chronic disease management while maintaining independence and comfort at home.
You will manage a caseload from admission to discharge, conduct assessments, and develop individualized care plans in collaboration with physicians and a care team. Daily travel within the service area is required.
