Primary Responsibilities

Case Manager:
Responsible for individuals in SVRC Vocational and Community Living Support programs. Provide support to individuals based on their plan of service.

Community Job Coach:
Provide individualized and small-group support services to individuals with disabilities to promote community inclusion, skill development, volunteerism, employment readiness, and successful participation in community-based activities.

Essential Job Functions Case Manager
  • Maintain assigned caseload and ensure participant services, documentation, reviews, and communications are completed within required time frames.
  • Responsible for vocational rehabilitation program participants, authoring designated plans of service, implementing and/or coordinating the implementation of the plan, writing progress reviews of the plan, communicating with the designated referral contact person and/or guardians about the plan and progress.
  • Responsible for special assignments within Vocational and Community Living Support programs, as assigned by supervisor.
  • Complete intake and orientation of participants to program or for services to be provided; may include tours, conveying of rules and expectations, signing necessary documents, conducting initial interview, and eliciting participant's cooperation and interest in the program.
  • Implement person‑centered plan, addendums, progress reviews at prescribed intervals.
  • Communicate (written and verbal) with referral contact person (and parent/guardian, if applicable) concerning participant's progress and/or problem development in a timely manner; inform supervisor of critical incidents immediately.
  • Coach participants on various work‑related topics such as appropriate grooming and dress, job seeking skills, work behaviors, how to impress a supervisor, behaviors which may lead to dismissal, potential jobs in the community, overcoming barriers, etc.
  • Coordinate services with participants, families, guardians, referral agencies, employers, and community partners as appropriate.
  • Provide follow‑along and support services to participants in community, volunteer, and employment settings to promote successful outcomes and goal achievement.
  • Transport participants as needed for program participation.
  • Communicate with job coaches or others on how to work with individual participants.
  • Maintain documentation in accordance with agency, CARF, and Community Mental Health standards and requirements.
Community Job Coach
  • Provide support services to participants in community, volunteer, employment, and recreational settings.
  • Build on participant's abilities and strengths based on: a) available past experience and information, and b) personal observations of the individual performing job tasks.
  • Provide on‑site/off‑site supervision of participants at all times unless otherwise indicated by the participant's plan of service.
  • Gain a high‑level knowledge of all elements of the assigned job(s), including general procedures and practices of the work site.
  • Implement participant service plans, goals, and support strategies as assigned.
  • Complete required service documentation, progress notes, transportation records, incident reports, attendance records, and other agency documentation accurately and within established timelines.
  • Track and document participant time and attendance.
  • Ensure participant health, safety, dignity, and well‑being while providing services in community and program settings.
  • Support participants in developing independent living, social, communication, and community participation skills.
  • Communicate participant progress, concerns, incidents, behavioral changes, and service needs to the appropriate supervisor in a timely manner.
  • Report incidents, accidents, participant injuries, and safety concerns in accordance with agency policy and established timelines.
