Hope Network, a non-profit empowering individuals with disabilities and disadvantages, is seeking a compassionate Case Manager in our Community & Social Services department. In this role, you will coordinate care, connect clients with essential healthcare and community resources, develop individualized service plans, and advocate for client needs. You will collaborate with an interdisciplinary team to support independence, recovery, and quality of life.

Responsibilities

  • Manage a caseload of clients receiving healthcare, behavioral health, neurorehabilitation, or employment services
  • Conduct intake interviews and comprehensive psychosocial assessments
  • Develop and implement person-centered service and care plans based on client strengths, needs, and goals
  • Coordinate services and appointments across medical, behavioral health, social service, and vocational providers
  • Advocate for client access to community resources, benefits, and support services
  • Support clients and families through care transitions and changes in level of service
  • Maintain timely, accurate, and compliant documentation in electronic health record and case management systems
  • Monitor client progress, update care plans, and adjust services based on changing needs
  • Participate in interdisciplinary team meetings, case conferences, and care reviews
  • Educate clients and families on available programs, rights, responsibilities, and self-advocacy skills
  • Assist clients in addressing barriers related to housing, transportation, employment, and healthcare access
  • Track outcomes and contribute to quality improvement and program development initiatives
