Job Overview

BAYADA Home Health Care is seeking a Home Health Speech Language Pathologist (SLP) to work in Saint Albans, Vermont. The position can be full-time, part-time, or per diem, with visits performed in senior living communities located in Williston, Vermont.

This role serves adult and geriatric patients in homes throughout Franklin and Upper Chittenden County, Vermont. Prior home‑care experience as an SLP is a plus but not required. As a key member of a multidisciplinary health‑care team, you will provide rehabilitative care and skilled nursing to enable clients to remain at home.

Responsibilities
  • Make outpatient visits to clients in designated senior living communities.
  • Perform thorough evaluations to assess, diagnose, and provide therapeutic interventions for clients experiencing speech, language, communication, or swallowing difficulties.
  • Develop a treatment plan based on the evaluation, physician’s orders, and recommended goals.
  • Continually assess and revise the speech‑therapy care plan, and participate with BAYADA clinicians in the multidisciplinary care plan, as appropriate.
  • Educate and instruct patients, family members, or other representatives in rehabilitative care and activities necessary to promote the client’s health, safety, and independent living.
  • Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided, utilizing a state‑of‑the‑art touch‑pad tablet.
Qualifications
  • Current license as a Speech Language Pathologist in Vermont.
  • Certificate of clinical competence in speech from the American Speech‑Language‑Hearing Association (ASHA) or equivalent educational and experiential requirements.
  • Solid computer skills; prior experience with electronic medical records (EMR) preferred.
  • Ability to work independently and as part of a team in a home‑care setting.
