Salary Range: $22.00 - $24.00 Hourly

Are you passionate about treatment planning and clinical documentation? Northwestern Counseling & Support Services is seeking a dedicated individual to join our Adult Home and Community-Based programming as a “Pod Lead.”

• Providing services across various community-based settings, focusing on supportive counseling, symptom management, daily living activities, social and interpersonal skills development, money management, and advocacy.
• Ability to build rapport with individuals who may be difficult to engage or resistant to treatment.
• Maintain a positive attitude, work well within a team, and stay organized to manage duties effectively.
• Collaboration and care coordination with community resources, agencies, and service providers.
• Develop and oversee treatment plans, service coordination, crisis intervention, and representative payee services.
• Being lead on treatment plan and associated documentation within a pod model.

• Reliable transportation with the capacity to transport clients in personal vehicle.
• Experience in mental health case management is a plus, but we are willing to train the right candidate.
• Bachelor’s degree in social work, psychology, public health, or related field; or equivalent combination of related education, training and experience.
• 1–3 years of experience in community mental health or care coordination, preferred.
