* Job Description:
** - Take pride in completing OASIS documentation efficiently and in line with Agency policies, State and Federal regulations.
- Utilize the nursing process to assess patient and caregiver needs, ensuring the delivery of exceptional care based on evidence-based practice.
- Develop and communicate a realistic plan of care for each patient, including active participation from patients and caregivers.
- Serve as the Case Manager for your assigned patients, actively managing your caseload and visit schedules to meet agency productivity standards.
- Implement the medical plan of care, nursing interventions, and procedures to guarantee high-quality, effective patient care.
- Stay updated on high-tech skills such as IV use and maintenance, wound VAC therapy, and complex wound care while upholding infection control standards.
- Collaborate with LPNs to ensure the care plan is effectively followed, delegate tasks properly, and ensure all patient needs are met.
- Evaluate and review various aspects of the implemented plan of care to ensure patient and caregiver needs are addressed.
- Maintain accurate and up-to-date documentation of nursing care provided to ensure effective communication across the healthcare team.
- Follow agency protocols related to McKesson processes, ensuring visits and follow-up notes are locked down and completed properly.
- Provide patient and caregiver education to empower informed decision-making regarding medications, treatment options, discharge instructions, and reportable conditions.
** Shift Information:** - Sunday - Thursday 8 AM - 4 PM with some on-call required.
