• Provide skilled nursing care to patients in their homes according to physician orders and individualized care plans
• Perform comprehensive patient assessments and ongoing evaluations of health status
• Administer medications, wound care, IV therapies, and other prescribed treatments
• Monitor patient progress and communicate changes in condition to physicians and healthcare teams
• Develop and implement individualized care plans to promote recovery and independence
• Educate patients and caregivers regarding disease management, medications, treatment plans, and home safety
• Coordinate care with physicians, therapists, case managers, and interdisciplinary healthcare teams
• Maintain accurate and timely clinical documentation in accordance with agency and regulatory standards
• Ensure compliance with patient safety, infection prevention, and quality care guidelines
• Provide compassionate, patient-centered care while promoting comfort and quality of life

• Active RN License in Vermont
• Minimum 2 years of recent Home Health nursing experience
• Current BLS Certification required
• Valid driver's license and reliable transportation preferred
• Experience with OASIS documentation required or preferred based on facility requirements
• Strong assessment, critical thinking, and clinical decision-making skills
• Excellent communication and patient education abilities
• Ability to work independently and effectively manage time and patient schedules
• Proficiency with electronic medical records and home health documentation systems
• Commitment to delivering compassionate, high-quality patient care
