Patterns Hiring is seeking a travel nurse RN Home Health for a travel nursing job in St Albans, Vermont

Hopsice coverage required when on‑call.
Responsibilities- Effectively fill out and complete OASIS documentation in accordance with Agency policies and procedures, State and Federal regulations, and keep up to date on changes.
- Utilize the nursing process to assess patient/caregiver needs and provide quality care in accordance with evidence‑based practice, State and Federal regulations, Vermont State Board of Nursing, and VNAHSR policies.
- Develop and communicate a realistic plan of care for each patient, ensuring patient/caregiver involvement.
- Serve as Case Manager for assigned patients, actively managing case loads and visit times in accordance with agency productivity standards.
- Implement medical plan of care, nursing interventions, and procedures to ensure safe effective quality of care.
- Stay up to date on high‑tech skills; IV use and maintenance, wound VAC, complex wound care, infection control standards.
- Collaborate with LPNs to ensure plan of care is followed, delegate tasks, review documentation, annotate visit notes and act on recommendations.
- Evaluate aspects of implemented plan of care, addressing problems, patient responses, and overall quality.
- Maintain accurate nursing documentation for integration with health care team.
- Follow agency protocol with McKesson processes: locking‑down visits, follow‑up notes, visit completion.
- Educate patients and caregivers on medication side effects, treatment options, discharge instructions, reportable conditions, and services.
- Identify inter‑disciplinary needs and communicate documentation to covered disciplines.
- Accurately fill all Agency, State and Federal forms (HHABN, Medicare Notice of Non‑Coverage) promptly.
- Carry out visits efficiently, ensuring supervisory visits not combined with routine visits when necessary.
- Assess patients’ needs for HCA services, coordinate PIC, and provide ongoing supervision.
- Transmit incident reports (infections, injuries) to QI Specialist promptly.
- Coordinate safe patient discharges and/or transfers, ensuring necessary paperwork and information processed.
- Coordinate patient care with physicians, providers, team members, vendors in support of plan of care.
- Participate in monthly peer review audits, give constructive feedback and accept feedback professionally.
- Schedule, coordinate, and facilitate case conferences with Manager and team members.
- Advocate for patient care issues to ensure overall quality is sensitive to each patient/family’s needs.
- 2 years of Home Health experience required.
- AHA BLS required.
