Overview

Registered Nurse – Home Health for a travel assignment in Saint Albans, Vermont.

Start date:

07/13/2026. Duration: 13 weeks. Weekly pay: $2,422 to $2,588. Guaranteed hours: 37 per week. Shift: 5x8 days. Setting: skilled nursing facility with home health services. This is a travel – temporary assignment under Healthforce Healthcare LLC.

Responsibilities
  • Conduct patient assessments using the nursing process and develop an individualized plan of care with patient and caregiver involvement.
  • Serve as Case Manager for assigned patients, actively managing caseload and visit times in line with agency productivity standards.
  • Implement medical plan of care, nursing interventions, and procedures to ensure safe, effective, quality care; maintain infection control standards throughout the plan of care.
  • Maintain up-to-date, accurate documentation (including OASIS where applicable), annotations on visit notes, and timely completion of required forms.
  • Collaborate with LPNs and other disciplines to ensure the plan of care is followed; communicate identified inter-disciplinary needs to relevant team members; provide patient and caregiver education.
  • Monitor and document patient responses to care, evaluate outcomes, and communicate recommendations to optimize ongoing quality of care.
Requirements
  • Proficiency in utilizing the nursing process to assess needs and develop care plans; ability to involve patients and caregivers in decision-making.
  • Ability to complete OASIS documentation in accordance with agency policies and state/federal regulations; stay informed on changes to requirements.
  • Capability to coordinate care with physicians, therapists, social workers, and other team members; effective communication and documentation skills.
  • Familiarity with high‑tech nursing skills (IV use and maintenance, wound care, infection control) and adherence to Vermont State Board of Nursing and agency policies.
  • Willingness to work in a home health setting within a skilled nursing facility context and participate in on‑call rotation with hospice coverage as required.
  • Strong organizational skills to manage caseloads, visits, and timely documentation.
