LVN ​/ LPN – LTC​/SNF

Long-Term Care Nurse

Provide skilled nursing care to residents in a long-term care or skilled nursing facility setting. Ensure patient comfort, safety, and adherence to established care plans while maintaining compliance with facility and regulatory standards.

Responsibilities:

• Administer medications and treatments accurately per physician orders.
• Monitor and document patient vital signs, symptoms, and progress.
• Assist residents with activities of daily living, including bathing, dressing, and mobility.
• Observe and report any changes in patient condition to nursing staff or supervisors.
• Follow infection control protocols, including COVID-19 vaccination and exemption documentation.
• Maintain a clean, safe, and respectful environment for residents.

Qualifications:

• Valid LVN/LPN license in the state.
• Reliable transportation and availability for every other weekend shifts.
• Strong knowledge of long-term care and skilled nursing procedures.
• Ability to complete necessary vaccine documentation or exemption forms.
• Excellent communication and team collaboration skills.
