LVN ​/ LPN – LTC​/SNF

Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN)

Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) needed for local long-term care and skilled nursing facility assignments. This role involves providing quality patient care within a supportive healthcare environment.

Responsibilities:

• Administer medications and treatments according to physician orders.
• Monitor patient vital signs and overall condition regularly.
• Document patient care accurately in electronic health records.
• Collaborate with interdisciplinary team to develop and implement care plans.
• Ensure compliance with safety and infection control protocols.
• Maintain professional standards and uphold patient dignity at all times.

Qualifications:

• Valid LVN or LPN license in the state of practice.
• Proven experience in LTC or SNF settings preferred.
• Ability to reliably transport to the facility location.
• Strong communication and interpersonal skills.
• Certified in CPR and basic life support (BLS).
• Must have completed the COVID-19 vaccination or hold a valid exemption.
