LVN ​/ LPN – LTC​/SNF

Nursing Care Provider

Provide nursing care to residents in long-term care or skilled nursing facility settings. Ensure the delivery of safe, compassionate, and competent patient care in accordance with facility policies and healthcare regulations.

Responsibilities:

• Administer medications and treatments as prescribed by physicians.
• Monitor and document patient vital signs and health conditions.
• Assist residents with activities of daily living and personal hygiene.
• Collaborate with interdisciplinary team to develop and implement care plans.
• Maintain accurate and timely clinical documentation.
• Follow infection control protocols and ensure compliance with safety standards.

Qualifications:

• Valid LVN or LPN license in the state of practice.
• Minimum of 1 year of experience in LTC or SNF settings.
• Reliable transportation to accommodate local assignments.
• Ability to work every other weekend as required.
• Ability to provide or have completed the COVID-19 vaccination, or hold a documented religious or medical exemption.
• Strong communication and interpersonal skills.
