LVN ​/ LPN – LTC​/SNF

Job Summary:

We are seeking a dedicated LVN/LPN for a long-term care/skilled nursing facility. The ideal candidate will provide compassionate nursing care and support to residents in a dynamic healthcare environment.

Responsibilities:

• Administer patient medications and treatments in accordance with physician orders.
• Monitor and document residents' conditions, vital signs, and responses to treatments.
• Collaborate with interdisciplinary teams to develop and implement care plans.
• Ensure compliance with safety protocols and facility policies.
• Maintain accurate and timely documentation of care provided.
• Provide emotional support and education to residents and their families.

Qualifications:

• Valid LVN/LPN license and licensing requirements for the state.
• Proven experience in long-term care or skilled nursing facilities.
• Strong interpersonal and communication skills.
• Ability to work weekend shifts, including 7 am to 11 pm on weekends.
• Demonstrate knowledge of infection control and safety procedures.
• Flexibility to adapt to changing resident needs and schedules.
