CNA – LTC​/SNF

Job Summary:

Assist patients with their daily activities and provide essential support within a Long-Term Care/SNF setting. Ensure patient safety and comfort while maintaining compliance with facility protocols.

Responsibilities:

• Support patients with Activities of Daily Living (ADLs) including bathing, dressing, and feeding.
• Monitor and report patient conditions and changes to nursing staff.
• Assist with mobility and repositioning to prevent bedsores and ensure patient comfort.
• Adhere to infection control policies, including proper hand hygiene and use of PPE.
• Document patient care activities accurately and promptly.
• Maintain a professional and compassionate approach to patient interactions.

Qualifications:

• Certified Nursing Assistant (CNA) certification required.
• Proven experience working in LTC or SNF environments preferred.
• Ability to reliably commute with dependable transportation.
• Strong interpersonal and communication skills.
• Must have a valid  facility access.
• Must be fully vaccinated against COVID-19 or have approved religious/medical exemptions.
