Requirements:
- Local only
- Every other weekend required
- Reliable transportation required
Responsibilities:
- Provide patient care in accordance with established standards.
- Maintain clear communication with patients and team members.
- Monitor patient conditions and report changes as needed.
- Assist with daily living activities and mobility support.
- Document care provided and maintain accurate records.
- Participate in team meetings and training as required.
