CNA – LTC​/SNF

Job Summary:

Assist patients with daily living activities and provide essential basic care under the supervision of nursing staff in a long-term care or skilled nursing facility setting.

Responsibilities:

• Provide routine personal care, including bathing, dressing, and grooming.
• Assist with mobility and repositioning to promote patient comfort and safety.
• Support patients with feeding and hydration needs.
• Monitor and document patient conditions and progress accurately.
• Follow infection control procedures and maintain a safe environment.
• Communicate effectively with team members and report any changes in patient status.

Qualifications:

• Valid CNA license in good standing.
• Proven experience working in LTC or SNF environments preferred.
• Demonstrate strong patient care and communication skills.
• Ability to lift, move, and position patients safely.
• Adhere to vital signs and infection control protocols.
• Reliable transportation and flexibility to work scheduled shifts.
