Job Summary:

Provide specialized nursing care to residents in long-term care or skilled nursing facilities. Ensure the delivery of quality patient care while maintaining compliance with healthcare regulations and facility policies.

Responsibilities:

• Administer medications and treatments according to physician orders and facility protocols.
• Monitor patient conditions and report any changes to the healthcare team.
• Document patient assessments, care plans, and progress accurately and thoroughly.
• Assist residents with activities of daily living, ensuring comfort and safety.
• Maintain a clean and safe environment, adhering to infection control standards.
• Follow all facility policies regarding identification, personal protective equipment, and vaccination requirements.

Qualifications:

• Hold a valid LVN or LPN license issued by the state.
• Demonstrate proficiency in long-term care or skilled nursing environments.
• Complete a minimum of COVID-19 vaccination or provide a religious or medical exemption.
• Possess excellent communication and documentation skills.
• Ability to work independently and as part of a healthcare team.
