VNA & Hospice of the Southwest Region in Vermont is seeking an Intake Registered Nurse (RN) to manage timely assessment and processing of referred clients for home health and hospice services. The role follows a Sunday–Thursday schedule, 8am–4pm, with a focus on timely response, documentation, and coordination with physicians and referral sources.
The ideal candidate will have BSN preferred, relevant home health/hospice experience, and strong communication and team skills.
