Overview
Sub-Acute, Long‑Term Care is a sector of healthcare bursting with potential and work‑life balance flexibility (no on‑call or weekends). We are seeking a full‑time (40 hours per week) provider for our Cedar Ridge Center located in Sissonville, WV. This position can be filled by a nurse practitioner or physician assistant. The base salary range for this position is $104,000 – $120,000 per year, in addition to our no‑cap incentive compensation bonus program.

ResponsibilitiesThe Advanced Practice Provider reports directly to the Director of Clinical Practice and/or the Area Clinical Practice Manager. The Advanced Practice Provider collaborates on‑site with the Center Leadership Team for center‑specific goals and priorities. The Advanced Practice Provider collaborates with the Medical Director and/or Attending Physician for patient‑specific medical care.
The Advanced Practice Provider provides primary and urgent‑care services to nursing center residents in collaboration with the other medical providers in the center. The Advanced Practice Provider works collaboratively with Center Leadership to achieve Align Med Partners’ mission of improving the lives we touch through the delivery of high‑quality health care and everyday compassion.
QualificationsGraduate from an accredited School of Nursing program, licensed as a registered nurse, and certified as a Nurse Practitioner with a Master’s Degree in Nursing and concentration and specialty track of Older Adult and/or Family Nurse Practitioner (FNP). The Physician Assistant must be a graduate of an ARC‑PA accredited program and pass the PA National Certifying Exam (PANCE) to use the PAC designation.
A minimum of two years experience working with adult or geriatric population is preferred.
APRNs must hold an unrestricted professional license and DEA for each state in which they practice.
