Position Summary We are seeking a Board-Certified/Board-Eligible Primary Care Physician (MD/DO) to join our outpatient team in St. Augustine, Florida.
The physician will provide comprehensive primary care services in an outpatient-only setting, focusing on preventive care, chronic disease management, and overall patient wellness. This role offers a balanced schedule within a collaborative, patient-centered environment.
Primary Responsibilities- Provide comprehensive primary care services, including diagnosis and treatment of acute and chronic conditions.
- Perform routine check-ups, preventive care, and health screenings.
- Develop and manage individualized treatment plans.
- Collaborate with specialists and care teams to ensure continuity of care.
- Maintain accurate and timely documentation within the Electronic Health Record (EHR).
- Contribute to a collaborative clinical environment focused on quality patient outcomes.
- Education:
Doctor of Medicine (M.D.) or Doctor of Osteopathic Medicine (D.O.) from an accredited institution. - Certification:
Board Certified or Board Eligible in Family Medicine or Internal Medicine. - Experience:
Prior outpatient primary care experience preferred; new graduates are encouraged to apply. - Licensure:
Active Florida medical license or eligibility to obtain one.
