Position: Pharmacist Practitioner - Full Time

Overview

The Clinical Specialist Pharmacist works in collaboration with other members of the patient care team to manage patient care outcomes including emergency medicine, critical care, medication management, ambulatory care, infectious diseases, cardiology, and pediatric. This position provides drug information services and assists in solving patient condition and drug‑related problems. Assists with delivery of service regarding drug monitoring and drug regimen adjustments while working both directly and indirectly with the patient and providers.

Responsibilities
  • Accurately interprets, profiles and files prescribers' orders. Ensures all medications filled, checked and delivered in a timely manner.
  • Monitors drug therapy for duplicates, contraindications, interactions, incompatibilities, allergies, and dosing errors.
  • Documents interventions in the electronic health record (EHR) professionally and accurately.
  • Review and monitors drug therapy that requires dosage adjustment per prescriber order. Intervenes with prescriber to clarify medication orders, dosage, frequency, route as needed. Documents changes in EHR as per verbal or telephone order.
  • Aseptically prepares intravenous fluids, irrigation solutions, additives, chemotherapy and other sterile products.
  • Maintains appropriate documentation. Dispenses control drugs and maintains accurate records and inventory as required by law and pharmacy policies.
  • Performs comprehensive medication therapy reviews for patients on assigned floors, assesses for medication‑related problems and introduces therapeutic solutions.
  • Documents order clarifications and interventions in the EHR.
  • Collaborates with nursing to ensure safe and effective medication use is being performed on assigned floors, available for questions, clarifications, safeguarding/troubleshooting automated dispensing cabinets and medication pumps in a professional manner.
  • Collaborates with prescribers to maintain accurate post‑discharge medication reconciliation and resolve medication related problems.
  • Provides training and education to staff pharmacists, interns and students regarding pharmaceutical care, pharmacoeconomics, clinical pathways and clinical guidelines. Provides drug information and in‑services to the hospital's professional staff as needed.
  • Perform all other duties as assigned by management within job scope.
  • Provides medication and disease state education to patients, both face‑to‑face during the patient's hospitalization and/or via telephone communication.
  • Participates on committee(s) as well as reviews/updates/creates evidence‑based protocols and policies.
Qualifications Education / Training

Degree/Diploma Obtained Program of Study Graduate ACPE Accredited School of Pharmacy

Preferences

PharmD or equivalent education.

Experience Requirements

2 - 4 years experience as a Clinical Pharmacist.

Preferences

Two to four years experience in a relevant practice area will be considered equivalent to each year of residency training. PGY2 residency or equivalent experience preferred. Emergency Medicine and hospital experience preferred, based on practice area of position.

Certificates / Licenses / Registration
  • Licensed Pharmacist in Florida
  • Basic Life Support (BLS)
  • Advanced Cardiovascular Life Support (ACLS)
Preferences

Board Certification by the Board of Pharmacy Specialties (BPS) in either Pharmacotherapy, Ambulatory Care, Infectious Diseases, Cardiology, Pediatric, Emergency Medicine or Critical Care.

Additional Information

BLS, provided by the American Heart Association (AHA), required within 30 days of hire. ACLS, provided by the American Heart Association (ACLS), required within 90 days of hire.
