MINIMUM QUALIFICATIONS:
- Experience working in a120-bed skilled
Long-Term Carefacility - Knowledge of Medicaid/Medicare eligibility or insurance processes.
- Must have excellent communication and interpersonal skills.
- Must be fluent in the English language: reading, writing, speaking.
- Must have basic skills in the use of Microsoft Office Outlook and Word.
- Must be willing to work evenings and weekends as needed.
- Mustbe willing toprovide management and coordinate case management, treatment, referred counseling and follow up services for residents.
- Must be a Florida Licensed Clinical Social Worker
- Successful completion of the employment screening process to include, but not limited to, a national background investigation and drug test.
- Knowledge of Health Insurance Authorizations & Eligibility & knowledge of Medicare

PERFORMANCE STANDARDS AND EXPECTATIONS:
- Works under the supervision of the Veteran’s Home Administrator
- Develop and implement policies and procedures for social services.
- Perform duties as backup to the Admission Coordinatorwhenneeded.
- Provides management,coordination and oversight for thesocialservicesdepartment.
- Monitors participation of the veteran resident in programs/services designed to meetpsychosocial,mentaland emotional needs of the veteran.
- Participates in Interdisciplinary Treatment team meetings to develop a care plan.
- Provide knowledge of Medicaid benefits to families and residents.
- Perform other duties as assigned.
