MINIMUM QUALIFICATIONS:

* Experience working in a 120-bed skilled Long-Term Care facility

* Knowledge of Medicaid/Medicare eligibility or insurance processes.

* Must have excellent communication and interpersonal skills.

* Must be fluent in the English language: reading, writing, speaking.

* Must have basic skills in the use of Microsoft Office Outlook and Word.

* Must be willing to work evenings and weekends as needed.

* Must be willing to provide management and coordinate case management, treatment, referred counseling and follow up services for residents.

* Must be a Florida Licensed Clinical Social Worker

* Successful completion of the employment screening process to include, but not limited to, a national background investigation and drug test.

PREFERRED

QUALIFICATION:

* Bachelor's degree.

* Knowledge of Health Insurance Authorizations & Eligibility & knowledge of Medicare

PERFORMANCE STANDARDS AND EXPECTATIONS:

* Works under the supervision of the Veteran's Home Administrator

* Develop and implement policies and procedures for social services.

* Perform duties as backup to the Admission Coordinator when needed.

* Provides management, coordination and oversight for the social services department.

* Monitors participation of the veteran resident in programs/services designed to meet psychosocial, mental and emotional needs of the veteran.

* Participates in Interdisciplinary Treatment team meetings to develop a care plan.

* Provide knowledge of Medicaid benefits to families and residents.

* Perform other duties as assigned.
