The Florida Department of Health in St. Johns County seeks a Human Services Program Specialist to provide comprehensive medical and non-medical case management services to clients living with HIV/AIDS through the Ryan White Program. The role involves conducting client assessments, developing individualized care plans, coordinating community resources and services, maintaining detailed records, and performing HIV testing and counseling. This position is part of an interdisciplinary team dedicated to ensuring continuity of care and supporting clients in navigating complex health and social systems.
Keyqualifications:
- Valid Florida driver's license and willingness to work in-office and field settings daily
- Knowledge of case management concepts, HIV/AIDS treatment and care, and interviewing techniques
- Skilled in written and oral communication, active listening, empathy, and conflict resolution
- Ability to utilize MS Office programs and database management systems
- HIV 500/501 certification required within 60 days of hire (Florida Statute 381.004)
- AIDS Education and Training Center (AETC) sponsored case management training required within six months of hire
