Title: Senior Certified Nursing Assistant (Position No. )
Location: Clyde
E. Lassen State Veterans' Nursing Home, Saint Augustine, St. Johns County, Florida
Salary: $37,420.76 annually (paid bi-weekly)
Pay Plan: Career Service – overtime eligible (shift differential may be available)
- Multiple positions, varied shifts, including weekends and holidays
- OPS SCNA Weekend Warrior program ($19.44/hour) Monday–Thursday 7 PM to 7 AM and all shifts Friday–Sunday (not full-time, no benefits)

- Valid Certified Nursing Assistant license in Florida
- Willing and able to work a schedule that includes weekends and holidays
- Able to work independently and as part of a team
- Effective communication in English (reading, writing, speaking, and understanding)
- Basic computer skills
- Successful completion of due diligence, including Level II background check and drug test
- Experience as a CNA in a 120-bed skilled long‑term care nursing facility
- Valid CPR/BLS certification
- High School Diploma or GED
- Assist residents with activities of daily living (ADLs) such as oral hygiene, bathing, dressing, grooming, hair and nail care, shaving, toileting, and assistance with meals and nutrition
- Observe, identify, and report resident concerns or needs to supervisors
- Provide escorts to resident appointments when required
- Perform restorative nursing measures outlined in individual care plans
- Apply simple nursing measures including splints, positioning devices, bedmaking, and linen changes
- Measure and record vital signs, intake and output as per care plans
- Monitor for potential skin breakdown or pressure ulcers and report to primary nursing staff
- Use appropriate techniques for mechanical lifts, transfers, turning, positioning, and range‑of‑motion exercises to reduce fall and pressure ulcer risk
- Provide appropriate end‑of‑life care and related assistance
- Follow facility policies, procedures, and dining room protocol during all care activities
- Respond to cognitively impaired or behaviorally challenging residents (e.g., dementia, Alzheimer’s), observe changes, and report to primary nurse/RN supervisor
- Ensure all resident safety devices are used per care plans
- Document all care on designated forms or electronic patient care system, preserving confidentiality
- Maintain efficient use of supplies and inventory tracking system
- Answer resident call lights promptly and meet resident requests in a timely manner
- Notify nurses of any changes in resident condition
- Complete assignments from primary nurse or RN supervisor within allotted time
- Attend all required facility training
- Perform other related duties as required
