We are seeking a highly organized and proactive Registered Nurse to support medical coordination, medication administration compliance, and staff training for approximately 80 residents receiving residential services. This is an on-site position and is not remote. Flexible scheduling is available. The ideal candidate will be comfortable managing multiple medical priorities, communicating with physicians and specialists, training direct care staff, and ensuring timely follow-through on appointments, referrals, diagnostics, labs, and medication orders.

This position focuses on care coordination, medical oversight, staff education, and follow-up management. It does not involve intensive skilled nursing care or routine hands-on clinical nursing duties. The Registered Nurse will play a key role in strengthening medical oversight across residential programs by developing reliable tracking systems, coordinating care with residential managers, and ensuring that medical recommendations are clearly communicated and implemented.

This position requires strong attention to detail, excellent communication skills, and the ability to work collaboratively with managers, staff, PCPs, specialists, labs, diagnostic providers, pharmacies, and insurance representatives.

The Registered Nurse will be responsible for tracking and monitoring medical appointments, diagnostic testing, lab work, referrals, and follow-up recommendations for approximately 80 residents. This includes creating and maintaining a tracking system to ensure appointments are completed, results are received, referrals are followed through, and recommendations from providers are communicated to the appropriate residential teams.

The RN will coordinate community medical appointments with residential managers, helping ensure residents attend appointments as scheduled and that necessary documentation, updates, and follow-up instructions are obtained. The RN will work directly with primary care physicians, specialists, laboratories, diagnostic centers, pharmacies, and other healthcare providers to support timely and appropriate care.

The RN will review new medication orders and medication receipts to ensure accuracy, clarity, and compliance with APD medication administration requirements. This includes confirming that managers and staff understand new medication orders, medication changes, discontinued medications, and related documentation expectations. The RN will also monitor medication administration practices to help ensure compliance, identify concerns, and provide corrective training when needed.

The RN will become an APD-approved medication administration trainer and validator, if not already credentialed. Once approved, the RN will teach the medication administration course, validate staff competency, and ensure staff meet all requirements for safe medication administration in accordance with APD guidelines. The RN will provide ongoing staff training related to diagnoses, medication administration, health monitoring, and resident-specific health needs.

The RN will also assist with triaging medical concerns or questions based on residents’ diagnoses, current health status, provider recommendations, and established care needs. This includes helping residential teams determine when concerns require provider follow-up, urgent care, emergency evaluation, documentation, monitoring, or additional communication with families, guardians, or healthcare providers.

The RN will help address insurance-related barriers that may delay care, including tracking pending authorizations, following up on denied or delayed services, and communicating with providers or insurance representatives to support timely access to appointments, diagnostics, labs, referrals, and prescribed interventions.

Review new medication orders, medication changes, discontinued medications, and pharmacy receipts.

Ensure medication orders are complete, clear, and compliant with APD regulations.

Ensure managers and staff understand new orders, medication changes, and documentation expectations.

Monitor medication administration compliance across residential programs.

Become an APD medication administration trainer and validator.

Teach medication administration courses and validate staff competency.

Train staff on resident diagnoses, medication changes, health monitoring, and health-related support needs.

Triage medical concerns and questions based on resident diagnoses, symptoms, provider instructions, and care needs.
