Location: St. Charles
Responsibilities
- Provide a range of primary care services to the patients of the Centre de santé Univi Health Centre (CSUHC); perform client assessment, follow‑ups, triage, screening, and counselling.
- Collaborate with nurse practitioners, physicians and other healthcare professionals to develop and implement individualized care plans.
- Participate in the development, delivery and evaluation of educational and health promotion programs.
- Maintain accurate and timely records and encounter data in patient electronic medical record (EMR).
- Participate in chart and case review.
- Communicate information clearly and concisely to patients and health care providers on a regular basis.
- Educate patients and their families about healthcare, treatments, and procedures.
- Participate in the assessment and reassessment of patient care needs.
- Adhere to policies and procedures as well as professional standards.
- Other duties as assigned by the Program Director.
- Bachelor’s Degree in Nursing, or Diploma with relevant experience.
- Membership in good standing with the College of Nurses of Ontario.
- Work experience in a primary care setting preferred.
- Knowledge and ability to assess the needs of various age groups.
- Experience in the planning, implementation and evaluation of various health‑related topics in a community setting.
- Superior problem‑solving skills with the ability to prioritize and time manage in a demanding office environment.
- Knowledge of community resources, especially in regards to health and social services.
- Ability to work within an interdisciplinary team.
- Current certification in Basic Cardiac Life Support (BCLS).
- Bilingualism is required (English and French).
- Solid working knowledge and ability to navigate a PC Windows environment, including shared drives and Microsoft applications.
- Valid Ontario driver’s license and access to a vehicle, would be an asset.
- Salary range: $65,169.10 – $81,927

- A criminal records check will be required prior to hiring.
