Social Services Director – Skilled Nursing Facility

Maple Grove Wellness & Rehabilitation in Fenton, MO is seeking a Social Services Director to oversee the psychosocial care of residents in our skilled nursing facility. In this leadership role, you’ll support the emotional, social, and mental well‑being of residents, ensuring smooth transitions and a high quality of life.

• Bachelor’s degree in Social Work or related field (Psychology, Sociology, Special Education, or Rehabilitation Counseling)
• Minimum of 1 year supervised social work experience in a healthcare setting with elderly populations
• Skilled Nursing Facility (SNF) experience preferred

• Complete psychosocial assessments and develop individualized care plans
• Maintain accurate, timely documentation to meet state and federal guidelines
• Provide emotional support and crisis intervention for residents and families
• Coordinate discharge planning and community referrals
• Advocate for residents’ rights and collaborate with the care team
• Educate staff and ensure compliance/survey readiness
