Signify Health, part of CVS Health®, seeks a Full-Time Clinician (Nurse Practitioner) to perform in-home health evaluations across a defined coverage area. Travel may be up to 100% with potential overnight accommodations as needed, and multi-state licensure will be required in some regions.
The clinician will conduct visits that include medication and medical history review, physical evaluation, and possibly diagnostic tests, using an iPad workflow.
