RISE Coordinator – Full Time – $23 per hour – Paid Weekly

Monday – Friday 8am-4pm

The RISE Coordinator will be at least 18 years of age, have at least a high school diploma (or equivalent) and 2 years of experience within the field of intellectual and developmental disabilities. An associate degree (or higher) in a relevant field of human services, education, social sciences or closely related field and supervisory experience in a human service setting is preferred.
Responsibilities- Research and develop monthly and daily calendars with a variety of meaningful activities, both on and off-site including physical, educational, arts, volunteering, and recreation.
- Assign client responsibilities to support staff—maintain a 1:3 ratio.
- Train new staff on Willows Way Core Values, day-to-day operations, client information, equipment, van operation, lifts, safety measures, etc.
- Map and establish goals with each client prior to the PCSP meeting, to assist client with identifying preferred goals for the following plan year. Finalize goals during the PCSP meeting.
- Complete client monthly reports.
- Assist with updating and maintaining current client files as mandated by Willows Way, CARF, DMH and DDRB to include releases, PCSPs, physician orders, etc.
- When needed, give medication(s) as prescribed and directed, complete all medication records according to Willows Way Policy and Procedures.
- Monitor client’s health and well-being, observe changes in emotions, mobility, physical appearance or patterns of living, document if needed.
- Additional duties as assigned.
