What You Will DoAs a Hospice RN Field Nurse, you will plan, organize, and direct home care services with an emphasis on community health education. Primary responsibilities include:
- Complete an initial assessment of patient and family to determine home care needs.
- Provide a comprehensive physical assessment and review the current and previous illness history.
- Supervise and direct the Licensed Practical Nurse (LPN) and/or Home Health Aide in implementing the patient care plan.
- Develop patient education plans and direct the LPN in their implementation.
- Create and modify the Home Health Aide care plan, supervising via patient feedback and onsite interactions.
- Evaluate and revise the plan of care as patient status and needs change.
- Use assessment data to determine nursing diagnoses and develop a care plan that includes therapeutic, preventive, and rehabilitative actions.
- Administer medications and treatments as prescribed by the physician.
- Identify discharge planning needs, develop the plan, and implement it prior to discharge.
- Act as Case Manager when assigned, coordinating patient care for the assigned caseload.
- Prepare clinical notes and communicate patient needs with the physician.

- One to two years of recent acute care experience in an institutional setting.
- Preferred:
Bachelor’s degree and one year of home health care experience. - Licensed driver with a personal automobile that is insured and in good working order, meeting state/organization requirements.
