What You Will Do as a Home Health RN Field Nurse

As a Home Health RN Field Nurse, you will:
- Complete an initial assessment of patient and family to determine home care needs. Provide a complete physical assessment and history of current and previous illness(es)
- Supervise and direct the Licensed Practical Nurse (LPN) and/or the Home Health Aide in implementing the patient care plan
- Develop patient education plans, as needed, and direct the LPN in implementing said plans
- Create and modify the Home Health Aide care plan and supervise them via patient feedback and onsite interactions
- Evaluate and revise plan of care as patient status and needs change
- Use health assessment data to determine nursing diagnosis
- Develop a plan of care, which establishes goals based on nursing diagnosis and incorporating therapeutic, preventive, and rehabilitative action
- Administer medications and treatments as prescribed by the physician
- Identify discharge planning needs as part of the care plan development and implement prior to discharge of the patient
- Act as the Case Manager when assigned by the Clinical Supervisor and assume responsibility to coordinate patient care for assigned caseload
- Prepare clinical notes as needed and communicate with the physician regarding patient's needs

Education, Licensure & Certification Requirements
- Graduate of an accredited school of nursing.
- Active RN licensure
- One (1) to two (2) years of recent acute care experience in an institutional setting
- Bachelor's degree with one (1) year of home health care experience preferred
- Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
