Location: St. Clairsville
Stonerise Home Health and Hospice is seeking a skilled Hospice Nurse Practitioner to join our Ohio team, providing compassionate home-based care to patients and families. You will conduct face-to-face visits, assess eligibility for hospice, document findings, and collaborate with the Hospice Medical Director to coordinate patient-centered care across our high-quality program.
Applicants must hold an NP license, MSN, have hospice experience preferred, and be able to travel daily.
