Job Title: Geriatric Rounding Physician – Community-Based Practice
Compensation: $225,000 – $250,000 annually

Position SummaryWe are hiring a Physician (MD/DO) who is passionate about senior care and enjoys working outside of the traditional clinic setting. You’ll round on-site at assisted living and memory care facilities, managing chronic conditions, coordinating care, and helping patients live fuller lives.
Key Responsibilities- Conduct patient visits within assigned communities across the metro
- Perform physical exams, diagnostics, and medication management
- Lead care discussions and long-term planning with residents and families
- Communicate regularly with advanced practice providers and facility staff
- Address complex conditions with a preventative, person-centered approach
- Current MN license as a physician
- Experience in geriatric medicine or primary care
- Background in Hospitalist or senior community rounding preferred
- Excellent interpersonal and leadership skills
- Ability to travel locally to communities with reliable transportation
