Job Title: Geriatric Rounding Physician – Community-Based Practice

Compensation: $225,000 – $250,000 annually

Position Summary:

We are hiring a Physician (MD/DO) who is passionate about senior care and enjoys working outside of the traditional clinic setting. You’ll round on‑site at assisted living and memory care facilities, managing chronic conditions, coordinating care, and helping patients live fuller lives.

Key Responsibilities:

• Conduct patient visits within assigned communities across the metro
• Perform physical exams, diagnostics, and medication management
• Lead care discussions and long‑term planning with residents and families
• Communicate regularly with advanced practice providers and facility staff
• Address complex conditions with a preventative, person‑centered approach

Qualifications:

• Current MN license as a physician
• Experience in geriatric medicine or primary care
• Background in Hospitalist or senior community rounding preferred
• Excellent interpersonal and leadership skills
• Ability to travel locally to communities with reliable transportation
