SummaryStep into a meaningful physician leadership role at Centra Care where clinical care, community partnership, and population health intersect. This position blends direct patient care with program leadership across coordinated care and correctional settings, offering the opportunity to collaborate closely with interdisciplinary teams and Centra Care leaders to shape strategy, improve care delivery, and impact outcomes for patients with complex medical, behavioral health, and social needs.
Schedule- Flexible FTE (0.4–1.0) | Onsite – Centra Care Midsota
- St. Cloud, MN - Day shifts | Flexible hours between 8am-5pm
- No call, holiday or weekend requirements
- Salary range: $260,000 - $300,000 per year
- Pay may be determined by factors such as education, experience, skills, knowledge, location, and internal equity
- Salary and salary range are based on a 1.0 FTE, reduced FTE will result in a prorated offer rate
- May be eligible for additional pay through bonuses, quality incentives, or production-based compensation.

Required:
- BE/BC in Family Medicine or Internal Medicine
- Experience working within Addiction Medicine
- Eligibility for Minnesota medical licensure
Preferred:
- Leadership experience in complex or vulnerable patient populations
- Interest in community health, population health, and systems-based care
