Clinical Ambulatory Pharmacist

Centra Care is seeking a Clinical Ambulatory Pharmacist to join our Neurosciences Pain Center team, providing comprehensive medication management services for patients with chronic pain conditions. In this role, you will work collaboratively with patients and the healthcare team to optimize medication therapy, improve quality of life, and support safe, effective, and patient-centered pain management.

As a Clinical Ambulatory Pharmacist, you will conduct comprehensive medication reviews, develop individualized care plans, provide ongoing follow-up care, and ensure therapies are appropriate, safe, effective, and convenient for each patient. Medication management may include both opioid and non-opioid treatment strategies, as well as addressing coexisting chronic disease states and medication-related concerns.

You will be part of a highly collaborative, interprofessional team consisting of physicians, advanced practice providers, and pharmacists who are committed to delivering holistic, evidence-based care. Success in this role requires strong clinical expertise, excellent communication skills, and the ability to build positive, trusting relationships with patients and colleagues across the care continuum.

What You'll Do

  • Deliver comprehensive medication management services and targeted follow-up care.
  • Assess medication appropriateness, effectiveness, safety, and adherence.
  • Develop and implement evidence-based treatment recommendations.
  • Collaborate with providers and care teams to optimize patient outcomes.
  • Educate and empower patients to actively participate in their care plans.
  • Document clinical interventions and outcomes within the electronic health record.
  • Bill for pharmacy services.
  • Contribute to quality improvement initiatives.

Schedule

  • Full-time | 64 hours per two weeks | Onsite
  • Days | Monday-Friday | Primary hours 8am-5pm
  • No weekends or holidays!
  • Pay and Benefits

    • Starting pay begins at $ per year and increases with experience
    • Salary range: $- $ per year
    • Salary range is based on a 1.0 FTE, reduced FTE will result in a prorated offer rate

    Qualifications

    • Doctor of Pharmacy (PharmD) or Bachelor of Science in Pharmacy required.
    • Current Minnesota pharmacist licensure or eligibility for licensure required.
    • Completion of a PGY1 Pharmacy Residency; PGY2 Ambulatory Care Residency preferred, or equivalent ambulatory care experience.
    • Board certification (or eligibility) preferred.
    • Basic Life Support (BLS) certification required.
    • Strong clinical, communication, and interdisciplinary collaboration skills.
