Direct Support Professional at Sevita:

Responsibilities

• Provide direct support with daily living activities (eating, grooming, mobility, and hygiene) while promoting independence
• Implement individualized service and behavior support plans as directed by supervisors and clinicians
• Support community integration by accompanying individuals to appointments, activities, and social events
• Ensure client safety, dignity, and rights at all times, following all policies and regulations
• Accurately document services, progress notes, and incident reports in a timely manner
• Assist with transportation using company or personal vehicle, following all safety procedures
• Monitor health and emotional well-being and promptly report changes to supervisors
• Maintain a clean, safe, and supportive home or community environment
• Collaborate with families, guardians, and interdisciplinary team members to support goals
• Participate in required training, certifications, and ongoing professional development offered by Sevita
