Description

We are currently seeking a full or part time Community Health Worker (CHW) at our Cook, MN clinic to assist patients with their health and social service needs. This position will become part of the care coordination, behavioral health and medical team responsible for giving comprehensive quality care to Scenic Rivers patients. The CHW will serve as an advocate for the health needs of individuals by communicating with healthcare providers, navigating the healthcare system, providing social support, resources and informal counseling.

The CHW will act as liaison or advocate and implement programs that promote, maintain, and improve individual and overall community health. This position will a ddress patient barriers to care by connecting them to relevant community resources, with the goal of enhancing patient health and well-being, increasing patient satisfaction, and reducing health care costs. Qualified candidates can receive on-the-job training while also obtaining an online Community Health Worker certificate.

Community

Health Worker Responsibilities:
  • Facilitate communication and utilize tools to help patients, families, groups and communities to navigate healthcare and social service resources
  • Coordinate patient access to community resources such as food, clothing, local housing, education, health insurance, and providers to teach life skills, prenatal classes, parenting skills, and mental health services
  • Educate healthcare stakeholders and providers about community health needs in order to address social determinants of health
  • Communicate patient care goals and objectives with colleagues and nursing team
  • Increase comprehension of health literacy by delivering culturally appropriate education to promote healthy lifestyle choices
  • Assist community residents in effectively seeking to remove health inequities such as poverty, discrimination, and their consequences, including powerlessness and lack of access to employment, quality education and housing, safe environments, and health care.
  • Determine eligibility and support enrollment of individuals in health insurance plans including MNsure Navigation
  • Conduct eligibility determination of services to the uninsured; coordinate transportation for the patients
  • Collaborate with community groups and participate with area agencies directed towards community physical, mental, and economic health of which SRHS is a member.
  • Conduct outreach to implement programs in the community that promote, maintain, and improve individual and community health.
  • Build connections and support vulnerable populations and healthcare providers
  • May collect data to help identify community health needs and provide information to stakeholders for program and policy improvements.
