Job SummaryCatholic Charities of the Diocese of St. Cloud is seeking a Homeless Outreach Worker to support our Transitional Housing program. This position is responsible for connecting resources to youth who are homeless or at risk of homelessness and providing a nonjudgmental approach to helping solve their crisis. The Transitional Housing Homeless Outreach Worker provides mobile case management and counseling to youth in crisis on the streets.
Whatyou will do
- Mobile case management/provide outreach services
- Respond to referrals coming to the program and make referrals when necessary to other programs
- Perform screenings/intakes with new referrals and outreach youth, explaining services to clients and complete all required paperwork.
- Maintain professional relationships with community resources
- Data collection and documentation of client interactions
- Adhere to the guidelines established by the program and funders
Bachelors' Degree in Education, Social Work, Psychology, Community Psychology, or related human services field
Experience2 years exp. working with youth; prefer experience in homelessness/mental health/chemical health.
Additional RequirementsMust be at least 21 years old. Must have a valid driver’s license and a working vehicle.
