Hospice - Nurse Practitioner - St. Cloud, MN

We are seeking a compassionate and experienced Advanced Practice Provider (APP) to join our dedicated Hospice Care team.

Summary
In this role, the APP will collaborate closely with the interdisciplinary hospice team to deliver high-quality, patient-centered end-of-life care. Responsibilities include providing direct patient care through comprehensive assessments, expert symptom management, and individualized treatment planning, while also serving as a clinical resource for hospice staff and external care teams to support continuity and excellence in care.

Schedule:

• Part Time .6 FTE | 48 hours every two weeks | Onsite Travel throughout Centra Care Hospice service area
• * Days| 8-hour shifts | 7:30am - 5pm
• On-Call every 4th or 5th week/weekend

Pay and Benefits:

• Salary range: $ - $139,880 per year
  • Pay may be determined by factors such as education, experience, skills, knowledge, location, and internal equity
  • Salary and salary range are based on a 1.0 FTE, reduced FTE will result in a prorated offer rate
  • May be eligible for additional pay through bonuses, quality incentives, or production-based compensation.

Qualifications:

• Required:
  • Must be BE/BC and able to obtain a MN License.
  • Current licensure as a Nurse Practitioner (NP) in the state of Minnesota
  • Valid driver's license and reliable transportation
• Preferred:
  • Experience in Hospice or end of life care preferred
