Case Manager Registered Nurse

Join the inpatient Care Management team ud Hospital as Registered Nurse Case Manager.

In this role, you will collaborate with an interdisciplinary team to facilitate patient care. You will be accountable for a designated patient caseload and plan care effectively to meet patient/family transitional needs, monitor length of stay, and promote efficient utilization of resources. You will facilitate collaborative management of patient care across the healthcare continuum, intervening as necessary to remove barriers to timely and efficient care delivery and reimbursement.

• Full‑time | 80 hours every two weeks | Onsite
• Day shift | Monday – Friday | 7:30 am–4 pm
• Occasional weekend & holiday rotation

• Pay rate minimum: $42.37 for BSN; $43.43 for MSN; pay determined by experience
• Earn extra pay for working weekend shifts
• $3,500 hiring bonus and $5,000 relocation bonus for eligible candidates

• Relocation assistance: must reside > 50 miles from St. Cloud Hospital and relocate within 30 miles upon hire

• Graduate from a nursing program
• BSN or higher degree in nursing required
• Two or more years of clinical experience as an RN required
• Registered Nurse in Minnesota required
• Current American Case Management Association (ACMA) Certification – RN required or obtained within five years
