The Centra Care Senior Transitions team brings comprehensive, patient-centered care directly to individuals living in assisted living, memory care, long-term care, and transitional care settings — meeting patients where they are, in every sense of the phrase.
Summary
Our interdisciplinary team works closely with patients, families, and fellow healthcare providers to deliver care that is preventative, proactive, and tailored to each person's unique needs. Together, we manage chronic conditions, address social determinants of health, support care transitions to and from inpatient settings — all within an integrated, collaborative model.
As a provider on this team, you'll travel to assisted living, memory care, and long-term care communities across Central Minnesota, supported by a dedicated team of clinical operations and administrative colleagues working remotely alongside you. It's a role that offers autonomy, variety, and the deep satisfaction of building meaningful relationships with some of the patients who need it most.
- Provide on-site primary care to patients located in the long term care facilities in Melrose, Sauk Centre, Long Prairie, and assisted living in Long Prairie.
- Manage a panel of approximately 130 to 150 patients across five buildings
- Clinical support from Clinical Assistants and Care Coordination through LPNs, RNs
- Access to diagnostics such as labs and x-rays as needed
- Collaborate with community partners within home care, hospice, and other resources as appropriate
Schedule
- Full-time | 72 Hours
- Days Monday-Friday | 8-hour shifts | 7:30am – 5pm
- Flexibility to design and manage your schedule
- Call: 1:4 Weeks coverage from home overnight, weekends, and holidays for seven days approximately every four weeks
Pay and Benefits
- Salary range: $ - $139,880 per year
- Pay may be determined by factors such as education, experience, skills, knowledge, location, and internal equity
- Salary and salary range are based on a 1.0 FTE, reduced FTE will result in a prorated offer rate
- May be eligible for additional pay through bonuses, quality incentives, or production-based (RVU) compensation.

Qualifications
Required:
- Graduate of an accredited Nurse Practitioner Program or Physician Assistant Program
- Board Certified/Board Prepared through AANP, ANCC
- Licensed or ability to obtain licensure through the Board of Nursing
- Ability to obtain and /or maintain DEA certification for Minnesota
- Proven ability to communicate effectively with strong verbal skills
- Excellent interpersonal and customer service skills
- Must have a valid and unrestricted driver’s license and minimum state-required liability auto insurance; local travel is required
Preferred:
- 2+ years of clinical experience as a Nurse Practitioner
