Nurse Case Management (PRC)

Join the Indian Health Service as a Nurse Case Manager in the Purchased/Referred Care Department at the Red Lake Service Unit in Red Lake, MN. Coordinate patient referrals, assessments, care plans, authorizations, and community resources, while supporting continuity of care and positive patient outcomes.

• Reviews, prioritizes, and coordinates patient referrals and case management requests.
• Conducts patient assessments, develops and monitors individual care plans.
• Coordinates care with patients, families, providers, insurers, and outside healthcare organizations.
• Obtains authorization and connects patients with appropriate healthcare and community resources.
• Maintains case management records, monitors patient outcomes, and prepares required reports.
• Serves as a clinical liaison and patient advocate during care team meetings and referral activity.
