Summary of Position

The Case manager is responsible for managing the VA Transitional Housing Per Diem program in Benton, Stearns, and Sherburne Counties, providing intensive case management services, including Targeted Case Management where eligible and available, to homeless Veterans subject to the project’s homeless definition. This is a full-time regular position with a workweek expectation of 40 hours.

Wage: $20.79-$25.26 per hour, 40 hours a week

Essential Duties and Responsibilities
  • Maintain a positive work atmosphere by behaving and communicating in a manner that fosters good relationships with co‑workers and supervisors. This includes, but is not limited to, resolving conflicts professionally, being courteous to staff, visitors, and clients, respecting others’ property and persons, and communicating professionally about co‑workers, supervisors, and subordinates. Support the Mission of The Salvation Army by treating every client, donor, and colleague with respect and dignity, and without discrimination of any kind.
  • Manage the Veterans Per Diem grant, including re‑applying for the grant and working with the bookkeeper to maintain accurate financial records.
  • Organize the yearly local program review completed by the local VA onsite, reviewing clinical records, fiscal data, veterans’ living space, building safety/security, nutrition inspection, and medication safety.
  • Adhere to The Salvation Army’s social services guiding principles as outlined in Faith In Action, emphasizing intentional outreach and holistic care for those served.
  • Provide initial and ongoing strengths and needs assessments, identifying barriers to permanent housing and developing plans with participants.
  • Provide case management to Transitional Housing participants for up to 24 months, engaging households in activities to increase skills, ensure housing stability, and maintain accurate client case files.
  • Provide case management to homeless veterans residing in the emergency shelter.
  • Advocate and assist with applications for entitlement benefits and mainstream resources, including economic assistance, medical assistance, Veterans’ services, SSI, and RSDI.
  • Provide crisis response—advocacy, planning, and intervention—to increase participant safety and prevent violence.
  • Prepare required statistical and financial documentation and forward it to the Social Services Coordinator as scheduled.
  • Outreach in conjunction with veteran homelessness outreach programs to identify eligible individuals who could benefit from the Transitional Housing program.
  • Work with participants to identify housing options that best meet their needs and preferences.
  • Build relationships with landlords, prepare participants for community and tenancy expectations, determine eligibility for rental assistance programs, help participants apply and comply, and support participants once housed to improve stability and longevity.
  • Address basic needs, build life‑management and independent‑living skills, address transportation needs, and assist participants in accessing and arranging support services and healthcare.
  • Provide behavioral and mental health support services—assessment, aftercare, and support—coordinate and/or attend mental health appointments, monitor symptoms and medication effects, and refer to Adult Rehabilitative Mental Health Services when appropriate.
  • Provide health and medical care services—monitor physical health, medication monitoring, care coordination, and HIV/AIDS/STI education and support.
  • Provide hands‑on employment instruction and skill building, employment counseling and coaching, and coordinate with vocational programs and employers.
  • Encourage appropriate educational opportunities for adults, providing mentoring and tutoring as needed.
  • Provide culturally‑competent and trauma‑informed services.
  • Attend trainings directed by the Social Services Program Coordinator.
  • Complete other jobs assigned by the Social Services Program Coordinator.
  • Respond to crises, disagreements, or other situations with clients as they arise throughout the building, using appropriate de‑escalation techniques when warranted.
