Responsibilities
- Provides social work services and discharge planning to patients and families
- Transitions patients to alternate levels of care

- A bachelor degree in social work from an accredited social work program
- Current State Social Work Licensure
- Possession of a valid motor vehicle operator’s license
- Proof of current motor vehicle insurance
Demonstrates expertise in providing social work services, including discharge planning and crisis intervention, while ensuring smooth transitions for patients to alternate levels of care. Holds necessary licensure and qualifications to support effective patient and family engagement.
