Lutheran Social Service of Minnesota is seeking a Case Manager to provide case management services to individuals in care or former participants living independently. You will coordinate with county social workers, foster parents, probation and other agencies to develop transition and housing plans, and assist with living skills curricula.
You will plan and facilitate bi-weekly meetings, provide housing and life skills support, market program services, and compile reports for program management.
