Regional Float Social Worker
Join the inpatient care management team as a regional float social worker. This unique role will have the ability to support multiple Centra Care hospitals as a float social worker.
Our Social Workers serve patients and families in our community and beyond by helping them during a challenging time in their lives. Our Social Workers enjoy the variety of cases they are presented with as there is always something new and different. You'll collaborate with other Social Workers to gain knowledge and experience and work with an interdisciplinary team of nurses, therapists, physicians, and dieticians.
You will have a designated patient caseload and will plan care to effectively meet the patient/family transitional needs including monitoring the length of stay and promoting community resources.
Schedule
- Full-time | 80 hours every two weeks | Onsite
- Day shift | Monday through Friday | 7:30am-4:00pm
Pay & Benefits
- Pay based on experience and degree
- Bachelor's in Social Work starting at $32.09 per hour
- Master's in Social Work starting at $33.15 per hour
- Pay range: $30.65 - $49.73 per hour
- $3,000 hiring bonus and $5,000 relocation bonus for eligible candidates!

Qualifications
- Current Licensed Social Worker (LSW) in Minnesota required.
- Bachelor's or Master's degree in Social Work required.
- 2 years or more of clinical experience as a Social Worker.
- A valid driver's license and a driving record in good standing required.
- American Case Management Association (ACMA) Case Manager-SW Certification required within five years.
